New Dental Patient Information Centennial
A Warm Welcome for Your Smile
We put in a lot of effort to be great hosts as well as dental professionals, which is why it’s so important to us that your initial visit goes well. You’ll find answers to basic questions below that should help you know what to expect when you come to see us, but if there is anything we haven’t covered below, be sure to contact our team with questions.
Your First Visit
After you have scheduled an appointment with us, our Office Manager will give you a call to gather your insurance information so she can give you a good estimate of what will and will not be covered during your visit. Once you’re here, you’ll be escorted to your treatment room and have a full set of intraoral photos taken before your teeth are cleaned.
You’ll then meet Dr. Shroff, who will ask what brought you to see us, go over your dental history, and walk you through the images from earlier. He’ll work with you to put together a personalized treatment plan based on your goals, and before you leave, you’ll have a printed timeline of your ongoing care that is fully tailored to you.
Dental Patient Forms
You can fill out our forms after you arrive for your appointment, or to save time, you can download and take care of them right now. Just remember to bring them to our office so we can check you in immediately!
